  HISTOPATHOLOGIC TYPE HISTOPATHOLOGICAL GRADE
Malignant Melanoma
Level I Confinement of the malignant melanoma cells to the epidermis and its appendages
Level II Extension into the papillary dermis, with at most, only a few melanoma cells extending to the interface between papillary and reticular dermis
Level III Extension of the tumor cells throughout the papillary dermis, filling it and impinging upon the reticular dermis without, however, invading it
Level IV Invasion of the reticular dermis
Level V Invasion of the subcutaneous fat
 

 
 
 


Low Risk Melanomas less than 0.76 mm thick and level II or III
Moderate Risk
  1. Melanomas less than 0.76 mm thick and level IV
  2. Melanomas between 0.76 mm and 1.5 mm thick
  3. Melanomas more than 1.5 mm thick and level III
  4.  
High Risk Melanomas more than 1.5 mm thick and level IV or V
